We also serve as an adult foster care (AFC) for elderly in need of direct care, nursing services and oversight, and care management in a safe, positive, and healthy environment. Our team strives to optimize the quality of living for all our residents through our patient-center practice and services. To meet all the needs of the seniors under our adult foster care program, we have a multidisciplinary team that will work together with the families and physicians involved to customize a comprehensive care plan. Our program aims to establish long-standing resources for adults in the communities and prevent or delay long-term placement.

What is Adult Foster Care (AFC)?
AFC is a care service provided to elderly adults and adults with developmental or physical disabilities. Given that these individuals could not live alone safely, AFC caregivers are trained to provide elderly adults with direct care on a day-to-day basis. Unlike the care given by a nursing home facility, AFC is given to patients in the comfort of their own homes. Caregivers provide them with the meals, companionship, assistance, and supervision they need.

AFC Member Eligibility
To be eligible as an AFC member, you must be:

  • 16 years of age or older
  • Incapable of living alone due to different medical, physical, cognitive, or mental condition.
  • In need of daily assistance with activities of daily living (ADL). This includes bathing, dressing, eating, toileting, etc.
  • Willing to live with your caregiver.
  • Qualified for MassHealth or capable of paying privately.
  • Approved for AFC services by a physician and/or Aging Service Access Point.

AFC Caregiver Eligibility
To be eligible as an AFC caregiver, you must be:

  • 18 years of age or older.
  • A family member or non-family member.
  • A person who is not legally responsible for the member (spouse, parent of a minor child, or a legally responsible relative).
  • Approved by MCS through an interview, references, and CORI check.
  • Able to provide 24-hour supervision to the member.

*A caregiver will receive a stipend for the care of the member. This will be excluded from taxable income.
