About This Program
This program provides intensive, family-centered support designed to stabilize placements and strengthen caregiving capacity when a family member has a disability. Services are tailored to meet each family’s unique needs, helping them build skills, access resources, and create a safe, supportive home environment.
Who This Program Serves
DCF-involved families whose household includes at least one member—child, youth, parent, or caregiver—living with a developmental, intellectual, physical, behavioral, learning, or sensory disability.
What We Provide
Our services are hands-on, consistent, and tailored to each family’s situation and goals.
- Intensive In-Home Support
3 sessions per week, primarily in the home & community. - Disability-Specialized Staff
Experience with IDD, autism/ABA, sensory, & physical disabilities. - System Coordination
Partners with schools, OT/PT, medical teams, DDS & DMH. - 2-Hour Emergency Response
Rapid clinical response for DCF emergency referrals.
